Statement of Registration - Winter Season 2026

The Statement of Registration Form is a requirement for all Clubs who utilise an active reserve and/or park and/or hard court in the City of Melville.

It is your responsibility to ensure all provided information is correct and true before submission.

Please read the following before commencing this form:

• This form must be submitted by COB 14 June 2026
• You must complete the required information for ALL players registered at your club.
• Information may be cross-referenced with your State Sporting Association.
• Future allocations may also be cross-referenced with your State Sporting Association.
• Incomplete applications will not be accepted.
